AB Introduction: Endodontics uses cell therapy strategiesto treat pulpal and periapical diseases. During thesetherapies, surgeons aim to reconstruct the natural microenvironmentsthat regulate the activity of dentalstem cells. Methods: We searched for more than 400articles in PubMed using key words from regenerativeendodontics and dental stem cell biology. In 268 articles,we reviewed what factors may influence histologicresults after preclinical dental treatments that useregenerative endodontic procedures after pulpectomy.Results: Several factors, such as the origin of stem cells,the biomimicry of scaffolds used, and the size of lesions,are considered to influence the histologic appearance ofthe regenerated pulp-dentin complex after treatments.Information is accumulating on transcription factorsthat generate the pulp-dentin complex and survival/trophicfactors that would benefit niche recovery and histologicresults. Conclusions: In this article, we discuss thenoninterchangeability of stem cells, the influence ofdentin-entrapped molecule release on pulp regenerationand survival of stem cells, and the need of positionalmarkers to assess treatments histologically. Theex vivo amplification of appropriate dental stem cells,the search for scaffolds storing the molecular diversityentrapped in the dentin, and the use of positional transcriptionfactors as histologic markers are necessary toimprove future preclinical experiments.
